HL7Cognitive currently stewards three HL7-balloted Draft Standards for Trial Utilization (DSTU) initiatives. Each offers a Service Functional Model (SFM) specifying the functional behavior of a service that is important for health information systems in general—and Clinical Decision Support (CDS) in particular.

HL7 Version 3 Specification: Event Publish & Subscribe Service Interface – Release 1 – US Realm consolidates messaging regarding clinical events, allowing different publishers to post and systems to subscribe and listen to events or “topics” of interest. The service provides a foundation for a wide variety of responsive, dynamic applications, including new result feeds, content syndication, subscriber and publisher presence related behavior and clinical workflow systems.

HL7 Version 3 Specification: Ordering Service Interface – Release 1 – US Realm provides a consistent, structured methodology for ordering a variety of services and products, such as pharmacy, nutrition, radiology, and laboratory items. It supports the lifecycle of an order and ensures that access controls to core functions are role based and appropriate, and that orders can be managed in a reliable and reproducible fashion. The service provides common governance and orchestration for the full order lifecycle, from a simple bed assignment order to a complex chemotherapy regimen involving multiple courses of treatment over many months.

HL7 Version 3 Specification: Unified Communication Service (UCS) Interface – Release 1 – US Realm facilitates communications between parties and enables efficient delivery and coordination of patient care. It provides standardized delivery of alerts, recommendations and other message-based notifications via email, SMS, IM or fax, as well as provides functionality for managing analog and digital voice communications like VoIP. The service supports dynamic routing and/or escalation of communication events to ensure that when intended recipients are not available, appropriate surrogates can be notified and priority messages can be responded to in a timely manner.

In many cases, a standard is a Service Functional Model (SFM) that describes the desired behavior that a service or technology is to perform. An SFM is essentially a set of functional requirements that should be supported by a vendor implementation. Under an agreement with the Object Management Group (OMG), HL7 focuses on functional requirements, which are then passed to OMG for developing a corresponding technical implementation specification.

OMG specifications build upon HL7 functional models by soliciting from the community a technology-neutral, platform-independent model, at least one platform-specific model, and a reference implementation that demonstrates the feasibility of the proposed standard.

Cognitive sponsors technical implementation specifications through OMG, namely for the HL7 Ordering Service SFM. In late 2015 championed the RFP proposal request at an HL7 Technical Meeting. The electronic health standards community, including Cognitive Medical Systems, will have until November 7, 2016 to respond with a technical implementation specification, which will be voted on by OMG member organizations.
